  7. Police Report (If Any)

Company Car

Company car requires the following documents:

  3. Company SSM (Borang B, Borang 9, Borang 24, Borang 49)
  4. Company SSM Borang 13 (If Company Name Amended)
  5. Company Stamp
  6. Driver’s Identity Card @ IC
  7. Driver’s Driving Licence
  8. Police Report (If Any)

Commercial Lorry

Commercial Lorry requires the following documents:

  1. Vehicle Registration Card @ VRC
